Welcome to the home web page of Dr.Luis F Anez MD PA. Our office is located in Mandarin and has been established for over 15 years. Our clinic provides ample experience in family practice, internal medicine, cancer prevention and treatment, diabetes, hypertension, obesity, cholesterol and thyroid disorders. The office tailors towards primary care and places emphasis on prevention and well being management. Currently, we have two physicians that are board certified in family practice and internal medicine. Our new associate Dr.Michael Hernandez is a graduate from Florida State University and is accepting new patients. Come to the office and experience primary care at its best.

Luis F. Anez, M.D., has been practicing medicine for more than 25 years and is Board Certified in family medicine. Much of his experience is geared towards service to the geriatric community in Jacksonville, Fl. He is fluent in Spanish.

Michael Hernandez, M.D., is a graduate of Florida State University, College of Medicine. He was previously practicing at Shands in Jackonville. He is Board Certified in Internal Medicine. While at Shands his main area of focus was in Chronic Disease Management / Geriatric. He is also fluent in Spanish.

Jenny Baltazar, ARNP-BC, is a graduate of the University of Florida. She is a Board Certified Advanced Registered Nurse Practitioner. She previously worked at Mayo Clinic as a MICU-RN.

Our office currently cares for patients at Specialty Hospital, Memorial Hospital, and Baptist Hospital. We follow our patients closely at many assisted living facility to make sure that their daily needs are tended to. The office also does domiciliary visits if needed. The office has a focused interest in the geriatric patient needs.
